Your health plan
When you apply for Healthy Louisiana, you will choose a health plan and may choose a PCP, for each family member who has Medicaid or LaCHIP. Your health plan will also provide any mental health and substance use treatment services you or your family members need.
If you do not choose a plan, Healthy Louisiana will choose one for you. Healthy Louisiana will send you a confirmation letter in the mail. It will tell you the health plan you are in.
How to change health plans:
In the first 90 days, you can change plans:
- Online, or
- By phone with Healthy Louisiana at 1-855-229-6848 (TTY: 1-855-526-3346). Ask to change health plans. Some agents speak Spanish. We have free interpreter services for other languages.
After the first 90 days
You can change plans for certain reasons. Reasons include your plan changing services you need, or you cannot get certain care you need. You can change health plans:
- On the Health Louisiana mobile app, or
- By phone with Healthy Louisiana at 1-855-229-6848 (TTY: 1-855-526-3346). Ask for a transfer form. Some agents speak Spanish. We have free interpreter services for other languages.
Once a year
You can change plans during open enrollment.
